Required Health Documentation

As part of the application process, we’re required to collect certain health documentation for each student. For immunization records, you’ll need to provide either a current vaccine record or complete a Request for Exemption from Immunizations for Reasons of Conscience form.

Additionally, Texas law requires certain health screens from schools, so we’ll need either a current sports physical or fill out an exemption affidavit. We respect each family’s choices regarding these health matters and simply ask that you provide the appropriate documentation for your situation during the enrollment process.
