Required Health Documentation

As part of the application process, we’re required to collect certain health documentation for each student. For immunization records, you’ll need to provide either a current vaccine record or complete a Request for Exemption from Immunizations for Reasons of Conscience form. Additionally, Texas law requires schools to screen students in grades 6 and 9 for spinal problems, so we’ll need either a current sports physical that includes this screening or a completed Spinal Screening Affidavit of Religious Exemption. We respect each family’s choices regarding these health matters and simply ask that you provide the appropriate documentation for your situation during the enrollment process.
